NICE NG239 applies to people over 16 and says to use locally validated laboratory thresholds when substantial variation exists. Other authorities and assays use different borderline windows. What can cause low vitamin B12?

Low intake is one pathway. B12 occurs naturally mainly in animal-derived foods and is added to some fortified foods, so people eating little or no animal food need a reliable fortified-food or supplement source. The goal is to correct deficiency and its cause—not to push the blood number as high as possible.
