Serum Folate Blood Test

A serum folate test measures folate, a B vitamin, in the liquid part of blood. It helps investigate possible deficiency, but recent intake can change the result. Read it with the laboratory鈥檚 own interval, the blood count and vitamin B12 context; one number does not establish the cause of symptoms.

Serum Folate Blood Test explained

A serum folate test measures folate, a B vitamin, in the liquid part of blood. It helps investigate possible deficiency, but recent intake can change the result. Read it with the laboratory鈥檚 own interval, the blood count and vitamin B12 context; one number does not establish the cause of symptoms.See reference 1,See reference 2,See reference 3

What the test measures

Folate is the general name for vitamin B9 compounds. It supports DNA production and cell division. Folic acid is a form used in supplements and fortified foods; a laboratory result is a concentration, not a record of how much folic acid you took.See reference 2

Testing is often part of an investigation of anemia or suspected vitamin deficiency. Folate and vitamin B12 are frequently measured together because both can affect red blood cells.See reference 3

Serum and red-cell folate answer different questions

Measurement
Serum folate
Useful context
Responds to recent food and supplement intake. It may not represent long-term status.See reference 2
Measurement
Red-cell (erythrocyte) folate
Useful context
Reflects a longer period, but laboratory variability can limit its value. A longer time window does not automatically make it the better test.See reference 1,See reference 2
Measurement
Blood count and B12
Useful context
Help put the folate result in context; neither is interchangeable with folate.See reference 1,See reference 3

Why there is no single universal cutoff

Use the range and comments on your own report. Laboratories can use different methods and reference intervals; a result outside an interval is a finding to investigate, not a diagnosis by itself.See reference 4

Two published examples, not personal targets

Source and setting
Mayo serum folate assay
Published interpretation
At least 4.0 碌g/L is its reference value; below 4.0 碌g/L suggests deficiency.See reference 1
Source and setting
NIH nutrition overview
Published interpretation
Serum folate above 3 ng/mL indicates adequacy in the cited nutritional assessment framework.See reference 2

Keep the number and unit together

For mass concentration, 1 碌g/L equals 1 ng/mL. A result of 5 碌g/L therefore has the same numerical value as 5 ng/mL. The two examples above use equivalent units but different interpretive cutoffs; the difference is not a conversion error.See reference 1,See reference 2,See reference 5

A report in nmol/L uses amount concentration, so its digits cannot be compared directly with ng/mL. Keep the original unit, assay and laboratory interval together when comparing reports. Unit conversion does not make two test methods identical.See reference 4,See reference 5

What can contribute to a low result

Low intake, reduced absorption, alcohol-related problems and increased needs can contribute to inadequate folate. Certain medicines also affect folate: examples include some antiseizure medicines and sulfasalazine. The relevant cause depends on the person, their diet, history and treatment.See reference 2

Folate deficiency can cause megaloblastic anemia, in which red-cell development is abnormal. Symptoms such as tiredness are nonspecific, and a folate result alone cannot show that folate is the explanation.See reference 2,See reference 3

What a normal or high result cannot prove

Recent food or folic acid can raise serum folate. An above-range result does not directly measure supplement toxicity, while a result within range does not settle every possible cause of anemia or neurological symptoms.See reference 1,See reference 2,See reference 3

Large folate intakes can improve the anemia of B12 deficiency without correcting its neurological damage. That is one reason clinicians consider B12 before attributing the whole picture to folate or selecting treatment.See reference 2

Preparation depends on the laboratory

Ask the collecting service about fasting and tell it about medicines and supplements. For example, Mayo鈥檚 serum assay requires an 8-hour fast and has specific cautions about recent methotrexate or other folic acid antagonists. These are assay instructions, not a universal rule or permission to stop treatment.See reference 1

Record when you last took supplements. If the result is unexpected, discuss sampling conditions and whether a repeat or other test would answer a useful clinical question.See reference 3,See reference 4

Questions that make follow-up useful

Ask which specimen and method were used, whether the blood count or B12 result changes the interpretation, and whether intake, absorption or medicines need review. Homocysteine can be influenced by folate, B12, kidney function and other factors, so it is not a folate-specific answer.See reference 1,See reference 2

Pregnancy planning has its own established folic acid guidance. A serum result is not a substitute for discussing that guidance with a maternity or primary-care professional, and this page does not choose a pregnancy dose.See reference 2

Keep these details with the result

Common questions

Is folate the same as vitamin B12?

No. They are different vitamins with connected roles in blood-cell production. Measuring one does not replace measuring the other when both are clinically relevant.See reference 2,See reference 3

Is red-cell folate always more accurate?

No. It reflects a longer period, but analytic variability matters. Mayo favors serum testing in its routine assessment; test choice belongs with the clinical question and laboratory.See reference 1,See reference 2

Does high folate mean I should change my supplement?

The result alone does not choose a dose or prove harm. Review the actual product, amount and reason for taking it with a qualified professional, particularly when B12 status is uncertain.See reference 2,See reference 3
