What ANC measures—and what it cannot tell you

Neutrophils are the most abundant white blood cell in many adults and form an important part of the early response to infection and tissue injury. ANC reports their concentration in circulating blood, usually as cells/µL or ×10^9/L.See reference 1,See reference 3

ANC does not measure neutrophil function, neutrophils stored in marrow or present in tissues, the condition of skin and mucosal barriers, or every other part of immune defense. ANC formula and units, step by step

If WBC is 4.0 ×10^9/L and segmented neutrophils are 40% with 2% bands, ANC = 4.0 × (40 + 2)/100 = 1.68 ×10^9/L, which is 1,680 cells/µL. If bands are not reported separately, do not invent them; calculate from the reported neutrophil component and label the result as an estimate.See reference 11

The number is the same when WBC is written as ×10^9/L, ×10^3/µL or K/µL: 4.0 in any of those formats represents 4,000 cells/µL. The cells/µL number is 1,000 times larger. Keep WBC and the resulting ANC in matching concentration units.See reference 1

Automated analyzers and reports do not always define immature neutrophil groups identically. When the laboratory already provides ANC, use that value instead of rebuilding it from rounded percentages.See reference 2,See reference 3

What can cause a low ANC?

A low ANC can be temporary after a viral infection or, less commonly, a severe bacterial infection. Other possibilities include chemotherapy or radiation, medicine effects, autoimmune neutropenia, vitamin B12 or folate deficiency, enlarged spleen, marrow failure or infiltration, and inherited neutropenia syndromes.See reference 2

Medicine review is essential because many drug groups can be involved, but the correct response depends on the drug, count, symptoms and reason it was prescribed. Depth is only part of risk. A stable personal baseline differs from a rapid fall after chemotherapy; brief mild neutropenia differs from prolonged severe neutropenia. Duffy-null and ACKR1/DARC-associated neutrophil counts: an important health-equity exception

A genetic variation affecting ACKR1 can cause the Duffy-null phenotype and a persistently lower circulating ANC. This is often called Duffy-null-associated neutrophil count (DANC); some guidance uses ACKR1/DARC-associated neutropenia (ADAN). What can cause a high ANC?

A high neutrophil count is called neutrophilia. Common reactive causes include bacterial or other infection, inflammation, tissue injury, surgery, burns, acute bleeding, physical or emotional stress and strenuous exercise.See reference 3,See reference 10

Smoking, pregnancy and medicines such as corticosteroids, epinephrine-like drugs or colony-stimulating factors can also raise neutrophils. A laboratory flag does not prove infection, and pregnancy-related elevation should be compared with pregnancy-specific context.See reference 8,See reference 10

A CBC with differential usually needs no fasting or special preparation, but another test ordered at the same appointment may have different instructions. Tell the clinician about prescriptions, over-the-counter medicines and supplements; do not stop them yourself.See reference 3

Recent infection, hard exercise, acute stress, smoking, pregnancy, corticosteroids, chemotherapy timing and growth-factor treatment can change the result or its meaning. Children and pregnancy need their own context

Neutrophil distributions change substantially through infancy and childhood. A child should be compared with an age-specific interval from the reporting laboratory or a validated pediatric source, not an adult web range.See reference 2

Pregnancy commonly raises neutrophil counts. In a large Oxford cohort, the pregnancy-specific neutrophil interval from 8 to 40 weeks was 3.7–11.6 ×10^9/L, higher than the study's non-pregnant interval. That population result is not universal, but it shows why pregnancy context matters.See reference 8

A pregnancy-related shift does not rule out infection or another cause. Symptoms, obstetric context, laboratory interval and the rest of the CBC remain essential.See reference 8

When a low ANC and fever need urgent care

Seek urgent medical assessment for fever or rapidly worsening infection symptoms with known severe neutropenia, recent chemotherapy, stem-cell transplant or major immunosuppression. Follow the exact fever plan given by the treating team. NICE defines neutropenic sepsis during anticancer treatment as neutrophils ≤0.5 ×10^9/L plus temperature >38°C or other clinically significant sepsis symptoms; oncology guidance also treats fever with neutropenia as time-sensitive. How can ANC change—and what treatment is evidence-based?

There is no universal diet, supplement or detox that safely raises a low ANC or lowers a high one. Granulocyte colony-stimulating factor, or G-CSF, has specific clinician-managed uses. The 2026 ASCO guideline recommends prophylaxis for selected people receiving cancer treatment when chemotherapy-related febrile-neutropenia risk and individual factors justify it.
